The Comparison of Clinical Evaluation of Ketamine+Pancurnium and Penthothal sod.+Pancuronium for Anesthesia Induction


Abstract
The author proposed the comparison of the anesthesia induction effects between Ketamine +pancuronium (group A) and pentothal sod.+pancuronium (group B) without Succinylcholine, because there are few patients who are contraindicated with Succinylcholine.
1. The methods of anesthesia induction in both groups are as follow:
a) The premedication was done with atropine 0.015mg/kg in the operating room immediate before induction.
b) In group. A ketamine 2mg/kg was given I.V. and followed pancuronium 0.08 mg/kg immediately, and after artificial ventilation of 100% oxygen for 1.5 minutes to 2 minutes endotracheal intubation was done.
c) In group B pentothal sod. 5mg/mg was given I.V., and following method of pancuronium and ventilation was same as done in group A.
2. Evaluation of induction patterns are as follow: Reaction 0, are estimated depent upon the condition of induction.
3. Smooth induction (Reaction O) was 65% in group A, but only 10% in group B.
4. The unsuitable induction (Reaction }*, was 35% in group A. but 90o in group B.
5. The capable causes of smooth induction in most of group A are effects of ketamine which bring anesthesia and initial effects of respiratory depression and supression of laryngeal reflex.
6. The causes of unsuitable induction in most of group B are effects of pentothal sod. which very likely brings the hypnotic and laryngeal spasm with increased response to upper airway stimulation plus insufficient dosage of pancuronium.
